Breeding Evaluation Entry Form (Conformation Show) & Breed Survey Entry Form
Registration for the 2023 USCA Mid-Eastern Regional Show
PLEASE READ ALL INFORMATION – the Links to the Entry Forms are at the bottom of the page.
Breeders may enter more than one Kennel Group. Stock Coat and Long Stock Coat Kennel Group(s) will be judged separately. Entry fee for a Kennel Group is $50. You can indicate and pay for your Kennel Group(s) on the entry form when entering a dog in the event.
Registration deadline: ON TIME ENTRIES due by June 4, 2023, THERE WILL BE NO EXCEPTIONS!
REMINDER: All paperwork and payment must be received by June 4, 2023. Entries that have not been paid will NOT be included in the catalog and WILL NOT be entered in the event. ORGINAL copies of all documents MUST be presented at checkin.
Please upload your documents when processing your entry. If that is not possible you have the option to go back to the entry form later and upload your documents until the entry deadline. OR, you may submit all your paperwork by email to cmandela@aol.com
** NEW THIS YEAR FOR THE REGIONAL ** Pictures of entries will be displayed on the online entry page. After you enter your dog, go back to the entry page instructions and click on the button that says ‘Click to Upload a Picture of Your Dog’. On the next page select the name of your dog from the drop down list and then upload your picture. This will look the same as we do for the USCA Sieger Show!
Dental Certifications will also be offered this year for a fee of $15. On the entry form in the ‘Dog Information’ section you can indicate if you will be doing a dental certification and the $15 fee will be added to your entry fee. During check-in you will receive your form to present for your dental certification. PLEASE NOTE: Only dogs 12 months old and older can have a dental certification done.
4-12 Month Classes
$50 for on time entry
- Proof of tattoo or micro-chip
- USCA, AKC, SV, CKC or other WUSV recognized registration & pedigree if possible
- Proof of current rabies vaccination
- Proof of membership in a WUSV recognized organization
12-24 Month Classes & Open Classes
12-24 month – $60 for on time entry
Open class $60 for on time entry
- Proof of tattoo or micro-chip (may be a previous signed show card)
- USCA, AKC, SV, CKC or other WUSV recognized registration & 4 generation pedigree
- Proof of current rabies vaccination
- Proof of membership in a WUSV recognized organization
Working & Veteran Classes
Working $75 on time entry
Veteran $10 on time entry
- Proof of tattoo or micro-chip (may be a previous signed show card)
- USCA, AKC, SV, CKC or other WUSV recognized registration & 4 generation pedigree
- Hip & elbow certification
- Breed Survey
- Copy of scorebook for proof of titles
- Proof of current rabies vaccination
- Proof of membership in a WUSV recognized organization
Breed Survey
Entry Fee $125 on time entry
- USCA, AKC, SV, CKC or other WUSV recognized club registration from the dog’s country of birth.
- USCA, AKC, SV, CKC or other WUSV recognized 4-generation pedigree with SV a-stamp or other WUSV accepted hip & elbow certification.
- For a USCA Breed survey the dog must be USCA registered with SV a-stamp or other WUSV accepted hip & elbow certification.
- Show Card with at least a “G” rating awarded by a USCA, SV or other WUSV accepted judge in a regular class over 12 months of age.
- Original Körschein for Breed Survey After Interruption, Lifetime/Re-Survey.
- Original scorebook for First Time Breed Surveys indicating passed AD and BH, and at least one IGP, IPO or SchH class title earned under a USCA, SV or other WUSV accepted judge with at least 80 in the C phase, or an HGH. Dogs with an HGH are not required to pass the AD or BH.
- Original proof of change or ownership.
